Job summary

Concentra in Phoenix, AZ is seeking an Occupational Therapist to join our patient care team. You will perform clinical duties, evaluate patients, plan treatment, and coordinate with the Center Therapy Director to deliver therapy services in line with Concentra protocols.

The role requires a Master’s in OT, state licensure, NBCOT, and BLS. Two or more years of hand, wrist, and elbow rehab experience is preferred, with a focus on high-quality, compliant patient care.

Qualifications

• Master's Degree in Occupational Therapy from an accredited program.
• Current state OT licensure and NBCOT certification.
• Valid BLS certification.
• Minimum two years' experience in hand/upper-extremity rehab preferred.

Responsibilities

• Deliver high-quality patient care and accurate evaluations.
• Perform treatment planning and coordinate with Therapy Director.
• Document care and ensure compliance with policies and regulations.
• Support multidisciplinary coordination and effective communication.
• Plan, implement, and interpret physical therapy programs for rehab.
• Assist with patient education and proper use of supports and exercises.
