Garden City Health Care Center in Modesto, CA is seeking a Certified Occupational Therapy Assistant (COTA) to join a dedicated rehabilitation team in a skilled nursing facility. This full-time role offers a supportive environment for both experienced clinicians and new graduates.

The COTA will assist the OT with patient care, implement treatment plans, and document progress while collaborating with an interdisciplinary team to optimize residents’ independence and quality of life.

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program and licensed in California.
  • Current, active California COTA license with required CEUs.
  • Experience in skilled nursing or geriatric settings preferred.
  • Proficient with electronic documentation systems (Casamba, PointClickCare).

Responsibilities

  • Treat residents as directed by the supervising OT.
  • Implement treatment plans per physician orders.
  • Record evaluations, daily and weekly notes, and recertification documentation.
  • Communicate resident progress with the OT and rehab team.
  • Assist nursing staff with Restorative Aide training as appropriate.
  • Participate in in-service training and care conferences.
  • Educate residents and families on maintenance and discharge planning.
  • Maintain a clean, safe treatment area and report equipment issues.

General Purpose

The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist with patient-related activities and provides direct patient care in accordance with physician orders, treatment plans, and applicable regulations.

Essential Duties and Responsibilities
  • Treat residents as directed by the supervising Occupational Therapist
  • Implement treatment plans according to physician orders
  • Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility policy, and payer requirements
  • Communicate resident progress, problems, and plans with the supervising OT and interdisciplinary team
  • Assist nursing staff with training of Restorative Aides as appropriate
  • Participate in in-service training programs and care conferences
  • Educate residents, families, and caregivers on maintenance programs and discharge planning
  • Participate in discharge planning and transition of care
  • Maintain a clean, safe, and organized treatment area
  • Report equipment issues to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, and staff
Supervisory Responsibilities

This position has no supervisory responsibilities.

Qualifications
  • Graduate of an accredited Occupational Therapy Assistant program
  • Skilled nursing or geriatric experience preferred
  • Licensure & Certifications: Current, active California Occupational Therapy Assistant (COTA) license; Must meet all continuing education requirements; Must maintain license in good standing with the State Board
  • Skills: Proficient in electronic documentation systems (Casamba and PointClickCare preferred); Strong communication, organization, and problem-solving skills; Ability to work independently and as part of a collaborative rehab team
Physical Demands

Frequent standing, walking, reaching, pushing, and pulling. Ability to exert up to 50 pounds frequently and 100 pounds occasionally. Occasional climbing, balancing, stooping, kneeling, crouching, or crawling. Visual abilities include close, distance, color, peripheral, and depth perception. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
