Job summary

Adaptive Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to deliver skilled visits in the Galveston area, working under an OT supervisor. This field-based role offers a full-time schedule Monday–Friday with a productivity-based pay model and a focus on patient-centered home health care.

You will perform ADLs/IADLs, safety/fall prevention, energy conservation, UE function training, and documentation, coordinating with the care team to ensure timely, high-quality care for

Qualifications

  • Must have active Texas COTA license and strong clinical judgment.
  • Ability to work independently in the field within supervising OT scope.
  • Excellent communication and thorough documentation skills.
  • Willingness to travel daily across assigned territory.

Responsibilities

  • Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
  • Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
  • Track objective functional progress and communicate updates to supervising OT.
  • Coordinate with nursing and broader care team; escalate changes in condition or safety concerns.
  • Document promptly and thoroughly.

Certified Occupational Therapy Assistant (COTA) - Home Health

  • Schedule: Full-time Monday–Friday

  • Location: Field-based (Galveston pod)

  • Comp: $58.00 per point

What you will do
  • Deliver skilled COTA visits in the home under the supervising OT’s plan of care.

Requirements

Must-haves

  • Active Texas COTA license (in good standing)

  • Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)

  • Strong communication and documentation

  • Current driver’s license and comfort with daily travel across assigned territory

Nice-to-haves

  • Prior home health experience

  • EMR experience
