Job summary

Adaptive Home Health in Denver is seeking a Certified Occupational Therapy Assistant (COTA) to deliver skilled home visits under the supervision of an OT. This full-time, field-based role requires daily travel across the assigned territory and supports a technology-driven home health model.

You will implement ADLs/IADLs, safety and energy conservation strategies, document in HCHB, and collaborate with nursing to adjust plans as needed.

Qualifications

  • Active Denver COTA license in good standing.
  • Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)
  • Strong communication and documentation
  • Current driver’s license and comfort with daily travel across assigned territory

Responsibilities

  • Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
  • Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
  • Track objective functional progress and communicate updates/concerns to the supervising OT.
  • Coordinate with nursing and the broader care team: elevate changes in condition and safety concerns.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Certified Occupational Therapy Assistant (COTA) - Home Health

  • Schedule: Full-time Monday–Friday + occasional weekends (as needed)

  • Location: Field-based

Adaptive Home Health is building a higher-acuity, patient-centered home health model across Denver. Our ultimate mission is to dramatically improve patient access to home health care.

The COTA role is the front line of that mission: you deliver high-quality functional visits in the home under the OT plan of care, helping patients stay safe and independent.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

Requirements

Must-haves

  • Active Denver COTA license (in good standing)

  • Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)

  • Strong communication and documentation

  • Current driver’s license and comfort with daily travel across assigned territory

Nice-to-haves

  • Prior home health experience

  • HCHB experience
