Job summary

H2 Health Prime Living in Destin, FL is seeking an Occupational Therapist to deliver 1:1 care within a luxury senior living community. You will help residents maintain independence and improve quality of life through goal-driven interventions and a continuity-focused care model.

This role offers flexible scheduling, a consistent caseload in a single community, and opportunities for professional growth across H2 Health's nationwide network plus in-house continuing education.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active or eligible state license to practice (Florida preferred).
• New graduates welcome; prior experience preferred.

Responsibilities

• Provide 1:1 occupational therapy care to residents in a senior living community.
• Help residents maintain independence and quality of life through goal-driven interventions.
• Collaborate with home health partners to ensure seamless transitions.

Are you looking to provide 1:1 care while seeing all your patients under one roof? H2 Health Prime Living has the pleasure to provide care to residents within a luxury Senior Living community in Destin!

As an Occupational Therapist on our team, you'll help seniors maintain independence, confidence, and quality of life through meaningful, goal-driven interventions. You'll also play a vital role in our care continuity approach, collaborating with our home health partners within the same communities to ensure consistency and seamless transitions for both patients and staff.

Education: A graduate of an accredited Occupational therapy program
