Job summary

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate with an interdisciplinary team, maintain documentation, and educate patients, families, and staff.

Qualifications

• Assess functional abilities in seniors to identify impairments and opportunities for improvement.
• Create evidence-based treatment plans tailored to SNF and post-acute care.
• Deliver OT interventions that promote independence, safety and quality of life.
• Collaborate with an interdisciplinary team to coordinate care and discharge planning.
• Maintain accurate and timely documentation of patient progress and outcomes.
• Educate patients, families, and staff on OT goals, strategies, and safety.

Responsibilities
