Job summary

Fifth Star Care Management is seeking a dedicated Occupational Therapist (OT) to join our in-house therapy team across two facilities: Three Rivers Health & Rehab and Northern Neck Senior Care Community. New grads are welcome to apply, with a $2,500 sign-on bonus for full-time hires.

The OT will conduct comprehensive evaluations, develop personalized treatment plans, and provide direct therapy services to improve residents’ independence and quality of life while supervising COTAs and

Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
• Current and valid Virginia license to practice Occupational Therapy.
• NBCOT certification is required.
• Previous experience in a long-term care or geriatric setting preferred; new grads welcome.
• Excellent interpersonal and communication skills.
• Strong organizational and time-management abilities.
• Compassionate and patient-focused approach to care.

Responsibilities

• Conduct comprehensive evaluations of residents' physical, cognitive, and psychosocial abilities.
• Develop and implement personalized treatment plans to promote independence in ADLs and functional mobility.
• Provide direct therapy services per physician orders and plans of care.
• Monitor progress and modify treatment approaches as needed.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.
• Participate in interdisciplinary team meetings and contribute to care planning.
• Educate residents, families, and staff on adaptive equipment and safety at home.
• Supervise COTAs and ensure compliance with standards.
• Collaborate with nursing and other therapy professionals for coordinated care.
• Adhere to regulations and company policies.

Position Summary

We are looking for an Occupational Therapist who is passionate about providing high-quality, resident-centered care and helping residents improve functional independence, participate in meaningful daily activities, and enhance their overall quality of life.

Key Responsibilities

• Conduct comprehensive evaluations of residents' physical, cognitive, and psychosocial abilities

• Develop and implement personalized treatment plans that promote independence in activities of daily living (ADLs), functional mobility, and leisure activities

• Provide direct therapy services in accordance with physician orders and established plans of care

• Monitor resident progress and modify treatment approaches as appropriate

• Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries

• Participate in interdisciplinary team meetings and contribute to care planning processes

• Educate residents, families, and staff on adaptive equipment, home safety, and strategies to support functional independence

• Supervise Occupational Therapy Assistants (COTAs) and ensure compliance with professional and regulatory standards

• Collaborate with nursing staff and other therapy professionals to provide coordinated, comprehensive care

• Adhere to all applicable regulations, ethical standards, and company policies

Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program

• Certification by the National Board for Certification in Occupational Therapy (NBCOT)
