Job summary

Select Home Health & Hospice (A Fortis Health Company) in West Lafayette, IN seeks an Occupational Therapist to join our home health team. You will evaluate patients, develop individualized care plans, and deliver therapy to improve function, safety, and independence in the home.

Qualifications

• Graduate of ACOTE-accredited OT program.
• NBCOT certification (or Medicare Conditions if licensed prior to 1978).
• Licensed OT in Indiana.
• Minimum of 1 year OT experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide OT services per physician's plan of care.
• Evaluate functional abilities and update care plans.
• Deliver interventions to improve function and independence.
• Monitor patient response and report changes to the physician and care team.
• Educate patients, caregivers, and staff on goals, techniques, and equipment.
• Reassess patient needs and revise plans as appropriate.
• Maintain accurate clinical documentation in compliance with policies.
• Collaborate with interdisciplinary team for coordinated, high-quality care.
• Participate in quality improvement initiatives and staff meetings.
• Provide supervision of the Occupational Therapy Assistant (OTA).

Job Summary

We are seeking a dedicated Occupational Therapist to join our team. This role plays a key part in helping patients achieve their rehabilitation goals and regain independence in their daily lives. The Occupational Therapist will work closely with patients and their families to develop and implement individualized care plans that support functional ability, safety, and overall quality of life within the home setting.

Duties/Responsibilities

• Provide occupational therapy services in accordance with the physician's plan of care.
• Evaluate and assess patient functional abilities and assist in developing and updating individualized care plans.
• Deliver therapeutic interventions to improve function, self-care, and independence, seeing multiple patients per day.
• Monitor patient response to treatment and communicate changes to the physician and care team.
• Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive equipment.
• Reassess patient needs regularly and revise plans of care as appropriate.
• Maintain accurate, timely clinical documentation in compliance with agency policies.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
• Participate in quality improvement initiatives, staff meetings, and ongoing education.
• Provide required supervision of the Occupational Therapy Assistant (OTA).

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.
