Job summary

Fortis Health is seeking a dedicated Occupational Therapist to join our in-home care team in Greenwood, IN. You will develop and implement individualized care plans to support functional ability, safety, and quality of life for patients in their homes.

You will work closely with patients and families, collaborate with an interdisciplinary team, supervise an OTA, and participate in ongoing education and quality initiatives. A valid driver’s license and NBCOT/ACOTE credentials are required.

Qualifications

  • ACOTE-accredited OT program graduate (required).
  • NBCOT certification (or Medicare POA if licensed before 1978).
  • Licensed OT in the applying state (required).
  • Minimum 1 year experience as an OT (required).
  • Valid driver's license, auto insurance, and reliable transportation (required).

Responsibilities

  • Provide OT services per physician's plan of care.
  • Evaluate patient functional abilities and develop care plans.
  • Deliver interventions to improve function and independence.
  • Monitor patient response and adjust plans with physician and care team.
  • Educate patients, caregivers, and staff on therapy goals and equipment.
  • Reassess needs and revise plans as appropriate.
  • Maintain timely clinical documentation per agency policies.
  • Collaborate with interdisciplinary team for coordinated, high-quality care.
  • Participate in quality improvement initiatives, staff meetings, and ongoing education.
  • Provide required supervision of the Occupational Therapy Assistant (OTA).

Job Summary

We are seeking a dedicated Occupational Therapist to join our team. This role plays a key part in helping patients achieve their rehabilitation goals and regain independence in their daily lives. The Occupational Therapist will work closely with patients and their families to develop and implement individualized care plans that support functional ability, safety, and overall quality of life within the home setting.

Duties/Responsibilities
  • Provide occupational therapy services in accordance with the physician's plan of care.
  • Evaluate and assess patient functional abilities and assist in developing and updating individualized care plans.
  • Deliver therapeutic interventions to improve function, self-care, and independence, seeing multiple patients per day.
  • Monitor patient response to treatment and communicate changes to the physician and care team.
  • Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive equipment.
  • Reassess patient needs regularly and revise plans of care as appropriate.
  • Maintain accurate, timely clinical documentation in compliance with agency policies.
  • Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
  • Participate in quality improvement initiatives, staff meetings, and ongoing education.
  • Provide required supervision of the Occupational Therapy Assistant (OTA).
Physical Requirements
  • Ability to sit, stand, and drive for extended periods while providing in-home care.
  • Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
  • Regular use of a tablet for documentation, communication, and training.

Qualifications
