Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area. The role involves evaluating abilities, planning and teaching daily living activities, and coordinating with the care team to ensure high-quality, patient-centered care.

We offer a flexible per-diem schedule with a pay rate of $110–$140 per visit. California licensure, CPR, and a valid driver’s license with reliable vehicle are required.

Formación

• Graduate of an occupational therapy curriculum accredited by major OT bodies.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and ability to travel to patient homes.

Responsabilidades

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living after illness or injury.
• Recommend adaptive equipment and instructional devices.
• Maintain accurate clinical records and report changes in condition.
• Coordinate with the care team for seamless patient-centered care.
• Educate family members and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition home.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
