Job summary

Boost Home Healthcare Fairfax West is seeking a compassionate Occupational Therapist to provide intermittent in-home therapy that improves independence and well-being. You will deliver skilled OT services per physician orders within an individualized plan of care, supported by our Client Services Director.

Ideal candidates hold an OT degree from an accredited program, are licensed or eligible for licensure, and have acute care plus home health experience.

Qualifications

• A passion to serve and help others live their best lives possible.
• Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the AMA and AOTA and/or certified by the National Certification examination.
• Eligible for the National Registration Examination of the AOTA.
• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply.
• Minimum of one year in an acute care setting.
• Two (2) years experience in Home Health preferred.
• Access to reliable transportation

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.
