Job summary

Trinity Health is seeking a compassionate Occupational Therapist to provide evaluations, treatment, and care planning for older adults across day center, nursing home, and in-home settings. Per diem, part-time position with collaboration in an interdisciplinary team.

You will conduct assessments, develop plans, and support home safety and adaptive equipment needs while educating participants, families, and staff on daily living activities.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited program.
• Current, active, and unrestricted OT license in the applicable state.
• Minimum one year of experience working with frail or elderly populations preferred; relevant clinical placement experience may be considered.
• Two years of occupational therapy experience preferred.
• Strong communication, collaboration, and clinical decision-making skills.
• Ability to work effectively in home, community, and clinical settings.

Responsibilities

• Conduct occupational therapy assessments and develop individualized care plans.
• Provide restorative, maintenance, and preventative therapy services.
• Evaluate home safety needs and coordinate home modifications and adaptive equipment.
• Train participants, caregivers, families, and staff on safe performance of daily living activities.
• Document assessments, treatment plans, progress notes, and outcomes in accordance with regulatory requirements.
• Monitor and maintain durable medical equipment inventory and utilization.
• Collaborate with the interdisciplinary team to achieve participant-centered goals.
• Supervise and provide guidance, competency assessments, and performance input for Certified Occupational Therapy Assistants (COTAs).
• Support quality improvement initiatives and uphold organizational values, compliance standards, and participant-centered care.

Home, or in-home setting.
