Trinity Health’s PACE program in Montgomery County seeks a full-time Occupational Therapist to evaluate and treat participants in Day Center, nursing home, and in-home settings. The LIFE of Montgomery County site will serve as the base for clinical OT evaluations and care planning.

Position Purpose: The primary purpose of this position is to provide clinical occupational therapy evaluation and treatment for the participants of the PACE Organization. This includes acute, restorative, or maintenance therapy as needed in various settings such as the Day Center, Nursing Home, or in-home settings.
