Job summary

Sonoma Post Acute in Sonoma, CA is seeking a dedicated Occupational Therapist to help residents achieve independence and improve their quality of life. You will evaluate residents, develop individualized treatment plans, and work with an interdisciplinary team.

The role requires a California OT license, prior experience in skilled nursing or rehab, strong clinical and communication skills, and a compassionate, patient-centered approach.
