Job summary

Millennium Physician Group is seeking a licensed Occupational Therapist to join our Home Health Team serving Charlotte and Sarasota Counties in a PRN capacity.

You will evaluate patients, develop and implement individualized treatment plans, educate families, and collaborate with an interdisciplinary care team to support independence and quality of life in the patients' homes. A Florida license and at least 6 months of relevant experience are preferred.

Qualifications

• Current and valid Florida Occupational Therapist license.
• Minimum 6 months experience in hospital, skilled nursing facility, or rehab.
• Strong communication, interpersonal, and critical thinking skills.
• Reliable transportation to visit patients at home.

Responsibilities

• Conduct comprehensive assessments of patients' physical, cognitive, and functional abilities to develop individualized treatment plans.
• Provide hands-on therapy to improve daily living skills, mobility, and independence; implement therapeutic interventions.
• Educate patients and families on strategies to manage conditions and maximize independence.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Document patient progress, treatment plans, and outcomes accurately and timely.
• Stay up to date with the latest research and advancements in occupational therapy.

As part of our compassionate, high‑performing care team, you will evaluate patients' physical, cognitive, and functional abilities, develop individualized treatment plans, and provide therapeutic interventions that promote independence and improve quality of life. You will educate patients and families, collaborate with an interdisciplinary care team, maintain accurate documentation, and deliver high-quality, patient-centered care.

This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call‑offs, vacations, or organizational requirements.

What You'll Do
