Job summary

ASSISTED HOME HEALTH, INC is seeking a skilled Part-Time/Per Diem Occupational Therapist (OT) to join our home health care team. You will assess patients in their homes, develop individualized treatment plans, and help patients regain independence in daily living activities.

Qualifications

• Active and unrestricted OT license in Florida.
• Minimum of 1 year of clinical experience (preferred in home health or rehabilitation).
• Strong communication and interpersonal skills.
• Proficient in electronic medical records (EMR).
• Ability to work independently and manage multiple patients.

Responsibilities

• Conduct comprehensive patient evaluations to assess physical, cognitive, and functional abilities.
• Develop and implement individualized treatment plans to improve daily living skills and independence.
• Provide hands-on therapy to improve strength, coordination, and mobility.
• Educate patients and families on techniques to promote safety and independence at home.
• Collaborate with physicians, nurses, and other healthcare professionals for coordinated care.
• Document patient progress and changes in condition in the EMR.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Adhere to agency policies and state/federal regulations.

Key Responsibilities:

• Conduct thorough patient evaluations to assess physical, cognitive, and functional abilities.
• Develop and implement individualized treatment plans aimed at improving daily living skills, fine motor skills, and overall functional independence.
• Provide hands-on therapy, including exercises and activities designed to improve strength, coordination, and mobility.
• Educate patients and their families on techniques and strategies to enhance independence and promote safety within the home.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
• Document patient progress, treatments, and changes in condition in the electronic medical record (EMR) system.
