Job summary

LewisGale Medical Center in Salem, VA seeks an Occupational Therapist to deliver high-quality, patient-centered care. You will manage the complete OT process for an assigned patient group, perform assessments, and develop individualized care plans in collaboration with the physician and care team.

Responsibilities include coordinating with the care team to reach goals, engaging patients in their care, and guiding the majority of the patient’s therapy across home health and continuum of care.

Qualifications

• Bachelor’s degree in Occupational Therapy.
• Licensed as an Occupational Therapist in the state(s) of practice.
• Basic Life Support (BLS) within 30 days of employment.

Responsibilities

• Manage complete occupational therapy care for assigned patients.
• Perform patient assessments and develop care plans.
• Coordinate with physicians and care teams to achieve goals.
• Provide or direct majority of patient’s therapy across home health and continuum of care.

Job Summary and Qualifications

The Occupational Therapist provides high quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Occupational Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience. The Occupational Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout home health and the continuum of care.

What qualifications you will need:

Required:

• Bachelor’s degree in Occupational Therapy
• Currently licensed as an Occupational Therapist in the state(s) of practice, in accordance with law and regulation.
• Basic Life Support (BLS) (within 30 days of employment)

Preferred:

• 1+ years experience in Occupational Therapy
• 1+ years experience in direct inpatient rehab
