Job summary

AdviniaCare is seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

As a Licensed Occupational Therapist, you will collaborate with care staff to address physical, cognitive, and emotional needs, educate residents and families, and monitor outcomes while maintaining accurate patient records.

Qualifications

• Licensed Occupational Therapist required.
• Ability to design and implement individualized therapy plans.
• Experience working with rehabilitation teams is a plus.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.
• Educate residents and their families on therapeutic exercises and techniques.
• Monitor and report on resident progress and outcomes.
• Maintain accurate and up-to-date records of patient care.
