Riverview Estates in New Jersey is seeking an Occupational Therapist to join our caregiving team and enhance residents' independence through skilled therapy.

You will perform comprehensive assessments, develop tailored plans, and collaborate with PTs and SLPs. RTM monitoring and detailed documentation in EMR are part of the role. We offer competitive pay and a supportive environment.

Qualifications

Responsibilities

• Section GG Data Collection: Ensure accurate data collection for annual and quarterly assessments and other metrics.
• Treatment: Administer OT interventions, including ADL training and environmental modifications.
• Education: Educate residents and families on safe mobility techniques and home exercises.
• Documentation: Maintain detailed evaluations, plans, notes in EMRs.
• Interdisciplinary Collaboration: Work with rehab staff for holistic care.

Responsabilities:

• Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
• Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
• Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
