Job summary

Riverview Estates in Riverton, NJ is seeking a dedicated Physical Therapist to join our team and guide residents through individualized treatment plans. You will work closely with licensed therapists to implement plans, monitor progress, and adjust care as residents advance toward mobility goals.

Responsibilities

• Assist the Physical Therapist in carrying out treatment plan and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
• Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
• Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with PoS care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMOs/Managed care progress updates in EMRs (Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through RTM with discharged patients to home or next level of care.
• Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.
