Job summary

Interim HealthCare Home Health and Hospice seeks a Home Health Occupational Therapist to help patients regain independence in home settings. You’ll work as part of a home health team to assess needs, set goals, and guide patients through therapeutic exercises and adaptive equipment to enhance daily living.

The role requires Michigan OT licensure, an accredited OT program graduate, and CPR certification, with flexible part-time hours paid by visit and a focus on compassionate, patient-centered

Qualifications

• Graduate of an accredited OT program with active Michigan OT license.
• Minimum 1 year OT experience, ideally in home health care.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgement and strong interpersonal skills; compassionate care.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks at home.
• Collaborate with home health team (RN, LPN, CNA, HHA, PT, SLP) to follow patient plan of care.
• Assess patient, set therapy goals, and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment (grab bars, shower chairs) to improve independence.
• Assess fall risks and implement home safety strategies.
• Educate patient and family on plan of care, exercises, and self-care.

As a Home Health Occupational Therapist, here’s a big-picture view of what you’ll do:

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

A few must-haves for Home Health Occupational Therapists:

• Graduate of an accredited Occupational Therapy Program and active OT license in Michigan
• Minimum of 1 year of occupational therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, resourceful and compassionate
