Job summary

Meadowview Rehabilitation & Nursing Center in Lafayette Hill, PA is seeking a compassionate PRN Occupational Therapist to join our in-house rehab team. This flexible PRN opportunity is ideal for an OT who is passionate about patient-centered care in a Skilled Nursing and Rehabilitation setting.

You will evaluate residents, develop individualized OT treatment plans, and provide skilled therapy to short-term and long-term residents within our SNF environment.

Qualifications

• Current OT license in the Commonwealth of Pennsylvania.
• Graduate of an accredited Occupational Therapy program.
• SNF/LTC experience preferred.
• Experience with Net Health and PCC is a plus.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services to short-term and long-term residents.
• Assess functional abilities, ADLs, cognition, and home safety needs for discharge planning.
• Collaborate with physicians, nursing staff, therapy team members, and families to provide coordinated patient care.
• Educate residents and caregivers on adaptive equipment, safety strategies, and therapeutic exercises.
• Complete accurate and timely documentation using Net Health and PCC and maintain regulatory compliance.

Meadowview Rehabilitation & Nursing Center is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our in-house rehabilitation team in Lafayette Hill, Pennsylvania. This flexible PRN opportunity is ideal for an OT who is passionate about delivering high-quality, patient-centered care in a Skilled Nursing and Rehabilitation setting.

As a valued member of our interdisciplinary team, you'll help residents maximize their independence, improve daily function, and achieve their rehabilitation goals.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.




• Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents.




• Assess functional abilities, activities of daily living (ADLs), upper extremity function, cognition, and home safety needs for discharge planning.




• Collaborate with physicians, nursing staff, therapy team members, and families to provide coordinated patient care.




• Educate residents and caregivers on adaptive equipment, safety strategies, and therapeutic exercises.




• Complete accurate and timely documentation using Net Health and PointClickCare (PCC).




• Maintain compliance with all federal, state, and facility regulations.

Qualifications

• Current Occupational Therapist (OT) license in the Commonwealth of Pennsylvania.




• Graduate of an accredited Occupational Therapy program.




• Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required.




• Experience with Net Health and PointClickCare (PCC) is a plus.




• Strong communication, clinical, and organizational skills.




• Ability to work independently while collaborating effectively with an interdisciplinary team.
