Job summary

HomeCentris Home Health is seeking an Occupational Therapist (OT) to provide quality care to Medicare skilled homecare clients in their homes. Flexible hours and a collaborative team atmosphere are highlighted. The position is based in Owings Mills, MD.

You will perform comprehensive assessments, coordinate physician orders, and deliver occupational therapy modalities to improve patient outcomes and patient satisfaction.

Qualifications

• Must hold a current Occupational Therapy license.
• Two years of skilled home health experience.
• Three years of healthcare experience.
• Four years of Occupational Therapy experience.

Responsibilities

• Performs accurate and comprehensive assessments so patients are safe and happy.
• Obtains physician orders for certification and recertification of home occupational therapy services.
• Coordinates care planning and delivery with the physician and other healthcare team members to optimize patient outcomes.
• Utilizes appropriate OT modalities for pain management.
• Other duties as assigned.

Duties:

• Performs accurate and comprehensive assessments so patients are safe and happy.
• Obtains physician orders for certification and recertification of home occupational therapy services
• Coordinates care planning and delivery with the physician and other healthcare team members and agency staff to facilitate optimal patient outcomes.
• Utilizes appropriate pain management modalities that include, but are not limited to, occupational therapy modalities.
• Other duties as assigned.

Requirements:
