Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist for per diem home visits across the Bay Area and beyond.

You will help patients regain independence and safety in daily living, delivering expert guidance directly in patients' homes with a supportive, family-oriented team. Responsibilities include evaluating home environments, creating personalized plans, and training patients in daily activities.

Qualifications

• Graduation from an occupational therapy curriculum accredited by the AMA and AOTA.
• Registered with the National Registration Examination (AOTA).
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Evaluate patients' functional abilities and home environments for safety.
• Develop personalized treatment plans and communicate with physicians and care team.
• Teach and retrain patients in daily living activities.
• Recommend adaptive equipment and devices.
• Maintain accurate clinical records and report changes.
• Coordinate with the care team for seamless patient-centered care.
• Educate family/caregivers on treatment approaches.
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service.
• Support discharge planning as patients transition home.
• Perform other duties as required for patient services.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient's treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

Requirements

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
