Job summary

LiveWell Homecare Agency in Colorado is seeking an Occupational Therapist (OT) to provide in-home healthcare services, evaluating clients and delivering therapy per physician orders. You will assess and plan patient care, supervise therapy assistants, coordinate services, and ensure compliance with state practice acts.

A valid driver’s license and reliable transportation are required to reach patients’ homes.

Qualifications

• Licensed or registered in Colorado as an Occupational Therapist.
• Graduate of an accredited Occupational Therapy program or eligible for certification.
• Minimum of one year of relevant experience.
• Valid driver's license, proof of insurance, and transportation.
