Job summary

Premier Home Health Providers Inc. seeks an Occupational Therapist to deliver in-home care on a per diem basis, helping patients regain independence and navigate daily activities in their own homes. You will assess needs, plan treatments, and coordinate with the care team to optimize recovery.

Qualifications

• Graduate of an accredited OT curriculum (AMA/AOTA joint).
• Registered with the NBCOT.
• Active California OT license.
• Minimum 2 years of clinical OT experience.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop personalized OT treatment plans and coordinate with physicians.
• Teach and retrain daily living skills.
• Recommend adaptive equipment and devices.
• Maintain clinical records and report changes promptly.
• Coordinate with care team for seamless, patient-centered care.
• Educate family and caregivers on treatment approach.
• Supervise OT Assistants and oversee home health aide care as needed.
• Support discharge planning as patients transition home.
• Perform other duties as required for patient services.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
