Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to provide per diem home health services in the Bay Area and surrounding California communities.

You will evaluate patients in their homes, design personalized treatment plans, and coach patients on daily living activities to regain independence and safety. Candidates must hold a California OT license, CPR certification, and have at least 2 years of clinical OT experience.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the appropriate authorities.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California OT license required.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $110 - $120 per visit.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
