Job Summary:Under a plan of treatment approved by a physician, assists the patient in attaining maximum potential through application of occupational therapy principles. Provides consultation to other health team members. Consults with patient's family and caregivers. Evaluates home environment and makes recommendations.

Education/Experience/Skills/Qualifications:

• Active Occupational license in California
• Current CPR certification
• Minimum of 1 year recent acute, SNF, or Home Health experience
• Current CPR certification

Special Requirements:

• Current driver's license
• Reliable vehicle
