Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to deliver comprehensive home health OT services in the Gahanna area. You will assess patients, develop and revise Plans of Care, and coordinate with nursing and therapy staff to ensure quality outcomes.

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical records.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in coordinating patient care.
• Use effective interpersonal relations and communication to coordinate with the care team.
• Assess physical, social, and cultural components of the patient’s occupational environment.
