Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to join our team in USA, TX Anna. You will assess patients, develop and maintain a Plan of Care, and coordinate ongoing treatment from admission through discharge.

The role emphasizes infection control, accurate documentation, and collaboration with clinicians. We offer Monday–Friday schedule, salary + vehicle allowance + on-call pay, full health benefits, PTO, 401(k), and professional development opportunities.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment and determine patient needs and care appropriateness.
• Document and update Plan of Care, progress notes, and other records.
• Revise the Plan of Care based on ongoing evaluation.
• Apply infection control and universal precautions in patient care.
• Accept clinical assignments aligned with training and competence.
• Use and promote effective interpersonal and team communication.
• Assess physical, social, and cultural aspects of patient's environment.
