Job summary

Elite Home Health is seeking a dedicated Occupational Therapist to provide in-home therapy across Florida. The OT evaluates patients, develops plans of care, and works with the interdisciplinary team to improve independence and safety in the home setting.

The role requires Florida OT licensure, CPR certification, and the ability to travel to patient homes. You will supervise OTA/HHA per policy and coordinate with physicians and nurses to optimize outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program and an active OT license in good standing.
• Current CPR certification is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform initial and ongoing patient evaluations to assess functional status and therapy needs.
• Complete comprehensive assessments and participate in OASIS-related coordination as applicable.
• Develop, implement, and update individualized occupational therapy treatment plans.
• Ensure physician orders are appropriate and aligned with the patient’s therapy needs.
• Provide skilled occupational therapy interventions in accordance with physician orders and established care plans.
• Instruct patients in safe use of assistive devices and guide home mobility and self-care activities.
• Teach and counsel patients and caregivers regarding therapy goals, home safety, and self-care strategies.
• Evaluate the home environment for modifications and coordinate outpatient services when needed.
• Complete timely clinical documentation, progress notes, and discharge summaries and ensure regulatory compliance.
• Supervise OTA and Home Health Aide per policy; participate in care conferences and discharge planning.

Position Summary

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.

The OT evaluates patient functional status, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve patient independence, safety, and quality of life.

Reporting Structure

This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The OT functions as part of the interdisciplinary care team and provides supervision to the Occupational Therapy Assistant (OTA) and Home Health Aide when required by the patient’s plan of care and agency policy.

Scope of Practice

The Occupational Therapist performs services within the scope of Florida OT licensure, Medicare Conditions of Participation, physician orders, and agency policies. The OT is responsible for evaluation, treatment planning, reassessment, and therapy supervision within the home health setting.

Essential Job Functions

1. Patient Evaluation & Care Planning

• Perform initial and ongoing patient evaluations to assess functional status and therapy needs
• Complete comprehensive assessments and participate in OASIS-related coordination as applicable
• Develop, implement, and update individualized occupational therapy treatment plans
• Ensure physician orders are appropriate and aligned with the patient’s therapy needs

2. Skilled Therapy Services

• Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
• Guide patients in therapeutic exercises, self-care activities, and functional mobility related to daily living
• Instruct patients in the safe use of wheelchairs, braces, splints, prosthetic, and orthotic devices
• Support patient safety, independence, and restoration of functional abilities in the home setting

3. Patient & Family Education

• Teach and counsel patients and caregivers regarding therapy goals, home safety, adaptive techniques, and self-care strategies
• Evaluate the home environment and identify modifications or community resources needed to support care
• Coordinate outpatient services when procedures cannot be safely performed in the home

4. Documentation & Compliance

• Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries
• Evaluate effectiveness of therapy interventions and update the plan of care as needed
• Maintain compliance with Medicare requirements, state regulations, and agency policies
• Communicate regularly with physicians, nurses, and interdisciplinary team members regarding patient progress and needs
• Supervise the Occupational Therapy Assistant (OTA) in accordance with agency policy and regulatory requirements
• Supervise the Home Health Aide when Occupational Therapy is the only skilled discipline involved in patient care
• Participate in care conferences, discharge planning, staff meetings, and required in-services

Qualifications

• Graduate of an accredited Occupational Therapy program required. Current state Occupational Therapist (OTR) license in good standing required. Minimum of one (1) year of experience in occupational therapy required; home health, acute care, or rehabilitation experience preferred.
• Current CPR certification required
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to pass background screening and all compliance requirements

Work is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
