Sunset Valley Rehabilitation & Healthcare Center in Littlefield, TX is seeking a PRN Occupational Therapist to join our in-house rehab program. The position supports geriatric clients across long-term and post-acute care settings within a collaborative team.

We offer a flexible PRN schedule and a focus on evidence-based, compassionate care in a supportive environment that values patient outcomes and professional development.

Qualifications

• OTA license is required; open to OTAs at all experience levels.
• New grads are welcome.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment to patients per regulatory requirements.
• Administer procedures as part of the rehabilitation plan including ADL retraining, activities, exercises, modalities, and use of devices.
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate treatment outcomes and refer as needed based on outcomes.
• Provide consultation to patients and families regarding physical disorders.
• Communicate with supervisor and health team about progress, barriers, and plans.
• Provide comprehensive discharge summaries per requirements.

Job description

Sunset Valley Rehabilitation & Healthcare Center in Littlefield, TX, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Occupational Therapist for our in-house rehab program!

Key Responsibilities:

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

Qualifications

Occupational Therapy Assistant license is required.
