Sonterra Health Center in San Antonio, TX is seeking a PRN Occupational Therapist to join our in-house therapy team. You’ll provide rehabilitative services in a post-acute setting across long-term and short-term care with geriatric outpatient options.

The role requires license as an Occupational Therapist and welcomes therapists at all experience levels. You’ll assess, plan, and deliver care aligned with regulatory standards while supporting patient independence and discharge planning.

Qualifications

• OT license required.
• Open to OTs at all experience levels.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative and medically necessary treatments.

Sonterra Health Center in San Antonio, TX, is currently seeking a PRN Occupational Therapist to join our in-house therapy team. We are a post-acute care facility with long-term care, short-term care, and geriatric outpatient services provided.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Occupational Therapist license is required.
• Open to OTs at all experience levels. New Grads are welcome!
