Job summary

Heritage Home Health and Hospice is seeking a motivated Occupational Therapist to provide skilled home health OT services. The role focuses on evaluating patients, designing individualized treatment plans, delivering interventions to improve daily living skills, and educating on home safety and adaptive equipment.

What We’re Looking For:

We are seeking an Occupational Therapist (OT) who is motivated, compassionate, and organized. The primary purpose of this position is to provide skilled occupational therapy services to patients in their homes, including:

• Evaluating patient functional needs and developing individualized OT treatment plans.
• Delivering interventions to improve independence with daily activities and overall quality of life.
• Recommending and educating patients on adaptive equipment and home safety strategies.
• Collaborating with physicians, nurses, and other members of the care team.
• Documenting care accurately in the electronic health record (EHR) system.

Home health experience is preferred but not required - we are happy to provide training and support to help you grow in this specialty. Strong communication skills, clinical expertise, and a commitment to patient-centered care are essential.
