The role focuses on evaluating patients, designing individualized treatment plans, delivering interventions to improve daily living skills, and educating on home safety and adaptive equipment.

You will collaborate with physicians and nurses, document care in the EHR, and support a flexible schedule within a patient-centered team dedicated to high-quality outcomes.

Qualifications

• OT license required.
• Strong communication and clinical skills.

Responsibilities

• Evaluate patient functional needs and create individualized OT treatment plans.
• Deliver interventions to improve independence in daily activities and quality of life.
• Recommend and educate patients on adaptive equipment and home safety.
