Job summary

Ohana Pacific Health seeks an Occupational Therapist in Lihue, HI to develop and implement progressive home health rehabilitation. The OT will serve as case manager, supervise HHA and COTA staff, and coordinate multidisciplinary care to meet patient goals.

Required NBCOT-accredited OT program, Hawaii OT license, BLS CPR, and 2 years of OT experience, preferably in home health. Includes a $2,000 retention bonus and relocation support.

Qualifications

• Completed accredited occupational therapy program recognized by NBCOT.
• Current Hawaii license to practice as an OT.
• Current Basic Life Support CPR certification.
• Two years of OT experience, preferably in home health.

Responsibilities

• Serve as case manager for assigned patients and supervise home health aides and COTAs when appropriate.
• Evaluate patients, admit those meeting criteria, plan and implement OT treatment.
• Perform reassessments, discharge when goals are met, or requirements are not met.
• Develop HHA and COTA care plans and delegate duties per policy.

The Position:

We are looking for a passionate Occupational Therapist (OT) to develop and implement a progressive rehabilitation program for patients in their homes in accordance with the principles and practices of occupational therapy and the patient’s needs. This position serves as the case manager and supervises home health aides and certified occupational therapy assistants when appropriate.

• Evaluates assigned patients. Admits those who meet agency and home health criteria.
• Plans, coordinates, and implements the appropriate treatment according to patient’s needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.
• Develop Home Health Aide (HHA) and Certified Occupational Therapy Assistant (COTA) care plans. Delegates duties to HHA’s and COTA’s based on agency and regulatory policies. Supervises and provides input to HHA's and COTA's per agency policy.
• Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.

Requirements:

• Have successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirements
• Current medical/health screening including 2 step TB clearance.
• Valid State of Hawaii license to practice as an Occupational Therapist.
• Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association
• Two years of experience as an occupational therapist, preferably in home health or other healthcare practice setting.
