Job summary

Fortis Health in Lebanon, IN is seeking a dedicated Occupational Therapist to help patients regain independence at home. You will develop and implement individualized care plans that support functional ability and safety within the home setting.

You will work with patients and families, supervise OTA, and collaborate with an interdisciplinary team while documenting accurately.

Qualifications

• Graduate of an accredited Occupational Therapy program (ACOTE or equivalent).
• NBCOT certification (or Medicare Conditions of Participation requirements if licensed prior to 1978).
• Licensed as an Occupational Therapist in the state which you are applying (required).
• Minimum of 1 year experience as an Occupational Therapist (required)
• Valid driver’s license, proof of auto insurance, and reliable personal transportation (required)

Responsibilities

• Provide occupational therapy services in accordance with the physician’s plan of care.
• Evaluate and assess patient functional abilities and assist in developing and updating individualized care plans.
• Deliver therapeutic interventions to improve function, self-care, and independence, seeing multiple patients per day.
• Monitor patient response to treatment and communicate changes to the physician and care team.
• Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive equipment.
• Reassess patient needs regularly and revise plans of care as appropriate.
• Maintain accurate, timely clinical documentation in compliance with agency policies.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
• Participate in quality improvement initiatives, staff meetings, and ongoing education.
• Provide required supervision of the Occupational Therapy Assistant (OTA).

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.
