Job summary

Trilogy Health Services in Lafayette, IN is seeking an Occupational Therapist to provide a full range of therapy services, including assessment, treatment planning and interventions in an interdisciplinary care setting at The Springs at Lafayette.

The OT collaborates with licensed clinicians to develop individualized treatment plans, educate patients and families, document progress and adjust plans to optimize daily living skills and functional independence.

Qualifications

• Education: Degree in Occupational Therapy from an accredited program.
• Experience: 0–1 years.

Responsibilities

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions to improve patients' functional abilities.
• Educate patients and their families on techniques and strategies for daily living activities.
• Document patient progress and adapt treatment plans as necessary.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
