Job summary

Synchrony Rehab in Louisville, KY is seeking an Occupational Therapist (OT) to provide a full range of therapy services, including assessment, treatment planning and interventions in an interdisciplinary environment.

The OT performs patient assessments, develops individualized plans, educates patients and families, documents progress, and collaborates with a diverse care team to ensure comprehensive patient care.

Qualifications

• Education: Degree in Occupational Therapy from an accredited program
• Experience: 0-1 years

Responsibilities

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions to improve patients' functional abilities.
• Educate patients and their families on techniques and strategies for daily living activities.
• Document patient progress and adapt treatment plans as necessary.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Licenses and Certifications

• Current, valid state OT licensure or registration
• Current valid CPR certification (preferred)
