Stern at Home Therapy is seeking a Part-Time Occupational Therapist to provide home health services across Jersey City, NJ and surrounding communities. The role emphasizes one-on-one patient care with strong clinical autonomy and a minimum commitment of 10 patients.

You’ll evaluate and treat a geriatric population in patients’ homes, helping maintain or regain independence with daily activities, while coordinating referrals and logistics with our administrative team.

Qualifications

• Active Occupational Therapist license in New Jersey required.
• Graduate of an accredited Occupational Therapy program.
• Reliable transportation and ability to work independently in home-based settings.
• Home health or geriatric experience preferred but not required.

Responsibilities

• Evaluate and treat geriatric patients in home settings with focused functional goals.
• Perform OT evaluations and develop individualized plans of care.
• Provide one-on-one OT treatments in patients' homes.
• Improve independence in activities of daily living.
• Address upper-extremity strength, ROM, cognition, balance, safety, and function.
• Educate patients and caregivers on exercises, strategies, and safety.
• Monitor progress and modify treatment plans as needed.
• Complete accurate clinical documentation and communicate with care team.
• Coordinate with physicians, caregivers, and families to optimize care.

Occupational Therapist (OT) – Home Health | Jersey City, NJ | Part-Time

Stern at Home Therapy is hiring a Part-Time Occupational Therapist (OT) to provide individualized, one-on-one care to patients throughout Jersey City, NJ, and surrounding communities.

This is an excellent opportunity for an Occupational Therapist seeking a consistent part-time caseload, flexible scheduling, and strong clinical autonomy. Our administrative team helps coordinate referrals, scheduling, authorizations, and patient logistics so you can focus on providing high-quality care.

With a minimum commitment of 10 patients, therapists can build a geographically concentrated caseload around their availability.

What You'll Do:

As an Occupational Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes, helping patients maintain or regain independence with daily activities and functional tasks.

Responsibilities include:

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one occupational therapy treatments in patients' homes.
• Help patients improve independence with activities of daily living.
• Address upper-extremity strength, range of motion, coordination, cognition, balance, safety, and functional performance.
• Provide therapeutic activities and exercises based on each patient's individual needs and goals.
• Recommend adaptive equipment, assistive devices, and home modifications when appropriate.
• Educate patients and caregivers on adaptive strategies, exercises, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.

Compensation:

Part-Time – Minimum 10 Patients

Part-Time W-2:

• $65 per follow-up visit.
• $75 per 4-unit evaluation.
• Fixed per-visit compensation with no tiered volume model.

Part-Time 1099:

• $72 per follow-up visit.
• $85 per evaluation.
• Fixed per-visit compensation with no tiered volume model.

Part-Time W-2 Benefits:
