Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC, is seeking an Occupational Therapist to join our long-term care team. This role supports residents with comprehensive OT evaluations, treatment planning, and interdisciplinary collaboration.

Qualifications

• OT degree in Occupational Therapy recognized by AOTA.
• SNF experience preferred.
• Current license/registration in the state.

Responsibilities

• Perform OT evaluations based on MD orders.
• Document findings on evaluation forms timely and accurately.
• Create comprehensive treatment plans with goals, frequency, and modalities.
• Obtain MD orders for evaluations, treatments, and discharges.
• Deliver therapy using modalities and adjust as needed while following precautions.
• Document daily participation and progress in progress notes.
• Update treatment plans monthly for recertification.
• Prepare discharge summaries when indicated.
• Document caregiver or discharge in-services as needed.
• Complete facility documentation forms as required.
• Contribute to MDS-related care plans and info.
• Attend facility and rehab meetings with supervision.
• Participate in facility committees as indicated.
• Act as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and performing screens.
• Bill patients accurately for services rendered.
• Cover for colleagues during absences as needed.
• Supervise junior staff, students, and technicians; co-sign notes.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education on patient care and OT services.
• Comply with facility policies and patient confidentiality.

Qualifications:

1. 1. Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
2. 2. SNF experience preferred
3. 3. Holds current license and/or registration in the state (as applicable)

Job Responsibilities:

1. 1. Provide a comprehensive occupational therapy evaluation based on MD orders.
2. 2. Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines.
3. 3. Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
4. 4. Ensure MD orders are obtained for evaluations, treatments, recertification and discharges.
5. 5. Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
6. 6. Document patient’s daily participation and progress on progress note accurately and timely.
7. 7. Document monthly updated treatment plan on recertification comprehensively and in a timely manner.
8. 8. Document patient’s discharge on a discharge summary as indicated.
9. 9. Document any caregiver or discharge in-services on an in-service form as indicated.
10. 10. Complete any and all other documentation forms required by Facility.
11. 11. Document care plans and information needed for the MDS as deemed necessary by the facility.
12. 12. Attend facility and rehab meetings as indicated by Supervisor and/or Regional Manager.
13. 13. Participate in facility committee meetings as indicated.
14. 14. Act as a rehab delegate to the interdisciplinary team.
15. 15. Participate in scheduling of patients as indicated.
16. 16. Perform screens as indicated.
17. 17. Complete billing and bill patients ethically and accurately for occupational therapy services rendered.
18. 18. Cover other therapists patient caseload during absence as necessary.
19. 19. Supervise junior staff members, students and technicians as needed. Co-sign notes as indicated.
20. 20. Maintain professional relationship with co-workers, facility staff, patients and patient families.
21. 21. Provide in-service education regarding patient care or occupational therapy services to rehab staff, facility staff, hospitals and/or and community as needed.
22. 22. Comply with all facility policies and procedures.
23. 23. Comply with patient confidentiality and Federal Resident Rights.
24. 24. Report to work on time, adhere to scheduled hours and project a professional image at all times.
25. 25. Adhere to productivity requirements.
26. 26. Comply with all Facility HR policies re: lunch, sick and time off notification.
27. 27. Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers.
28. 28. Perform any other duties assigned by the supervisor or Regional Manager.
29. 29. Adhere to any and all other Facility written and oral policies and procedures.
