Job summary

Coastside Adult Day Health Center in Half Moon Bay is seeking a part-time Occupational Therapist to join our multidisciplinary team. The role involves initial and ongoing assessments, treatment planning, and implementing person-centered activities to support independence and aging in place.

You will collaborate with the program director and other staff, complete quarterly assessments and routine progress notes, and contribute to OT policies and equipment recommendations.

Qualifications

  • Graduate of an accredited occupational therapy curriculum.
  • Registration with the American Occupational Therapy Association or eligibility.
  • Good work record, reliable and team oriented.

Responsibilities

  • Initial and ongoing assessment of participants and development of treatment plans.
  • Re-assessment and modification of goals as conditions change.
  • Deliver person-centered care and plan of care addressing participants' needs.
  • Provide input to multidisciplinary team for collaborative care planning.
  • Complete TAR/ IPOC every 6 months with progress notes.
  • Complete quarterly assessments with progress notes.
  • Complete and sign daily flow sheets with therapy details.
  • Promote independence and aging in place.
  • Enhance participants' emotional and social well-being.
  • Develop and guide use of therapeutic activities to improve function.
  • Advise on supplies and equipment for OT services.
  • Provide input on OT policies and procedures.
  • Other duties as assigned.

Job description

Job Title: Occupational Therapist

Reports to: Program Director

Location: Coastside Adult Day Health Center, 925 Main Street, Suite A, Half Moon Bay, CA 94019

Job Type: Part-time, 6-8 hours per week

This position requires a person who is a graduate of an accredited occupational therapy curriculum and is registered or eligible for registration by the American Occupational Therapy Association.

Duties
  • 1. Initial and continuing assessment of the participant, including the initial treatment plan, method, and interval of delivery of services to meet identified needs.
  • 2. Re-assessment of participants as conditions change, and modification of treatment goals consistent with these changes.
  • 3. Deliver person-centered care, building a plan of care addressing the participants' stated needs, goals, desires, and in consideration of their abilities and strengths.
  • 4. Provide input to the multidisciplinary team for collaborative care planning and delivery of services.
  • 5. Complete TAR/ Individualized plan of care every 6 months, including a progress note for that time period.
  • 6. Complete quarterly assessment every 3 months, including a progress note for that time period.
  • 7. Complete and sign daily flow sheets, indicating therapy received and participants' response to therapy.
  • 8. Increase and maintain participants’ capability for independence and aging in place.
  • 9. Enhancement of participants’ emotional and social well-being.
  • 10. Development of participants in the use of therapeutic activities for improving function.
  • 11. Guidance of participants in the use of therapeutic activities for improving function.
  • 12. Suggest the necessary supplies and equipment to conduct occupational therapy.
  • 13. Provide input regarding OT policies and procedures.
  • 14. Other duties as assigned.
Qualifications

A good work record, clean, neat, reliable, and capable of performing the above duties and others assigned. Good people skills, including: empathy and sensitivity to the needs of the elderly and disabled. Must also work well within a team model of care.

Pay Range: $40 to $80 per hour based on experience.
