Job summary

AgeRight Care at Home is hiring a Home Health Occupational Therapist in Gresham, OR. In this role you’ll deliver person-centered therapy in patients’ homes, assess safety and function, and collaborate with families and physicians to set achievable goals and progress plans. You’ll work with a flexible schedule and opportunities for advancement.

Starting pay is $45.00–$56.00 per hour. An Oregon OT license, CPR certification, and NPI are required, with comprehensive benefits including education

Qualifications

• Must be licensed as an Occupational Therapist in Oregon.
• Graduate from an accredited program with a degree in Occupational Therapy.
• CPR Certification.
• National Provider Identifier (NPI) required upon hire.

Responsibilities

• Establish treatment plan appropriate to physician orders.
• Prepare/complete clinical and progress notes.
• Teach patient and family/caregivers in meeting individual treatment goals.
