Job summary

Flagship Therapy in Gilbert, AZ is seeking an Occupational Therapist to join our in-house therapy team. This position offers full-time, part-time, or PRN options with a competitive hourly rate and sign-on bonus.

You will evaluate patients with cognitive, physical, and ADL deficits, develop individualized treatment plans, and collaborate with physicians and nursing staff. Thorough documentation and timely discharge planning are required.

Qualifications

• Graduate of an accredited Occupational Therapy program and licensed in Arizona or eligible.
• Arizona OT license or ability to obtain is required.
• New graduates are encouraged to apply and will be supported.
• Evaluate patients with cognitive, physical, and ADL deficits and create individualized plans.
• Use standardized assessments and evidence-based interventions.
• Provide occupational therapy per physician orders and regulatory requirements.
• Implement ADL retraining, therapeutic activities, adaptive equipment use.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing, families and the inter-disciplinary team.
• Educate patients and families about treatment plans and outcomes.
• Complete timely documentation and discharge summaries.

Education

Wellsprings of Gilbert, a premier short-term rehabilitation and skilled nursing facility in Gilbert, Arizona, is seeking a compassionate and motivated Occupational Therapist to join our outstanding in-house therapy team. We have Full-Time, Part-Time, and PRN opportunities available for clinicians who are passionate about helping patients regain independence and return home safely.

• $47 - $53 per hour based on experience

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in Arizona (or ability to obtain)
• New graduates and experienced therapists encouraged to apply
• Evaluate patients with cognitive, physical, and ADL deficits and develop individualized treatment plans
• Utilize standardized assessments and evidence-based interventions
• Provide skilled occupational therapy services in accordance with physician orders and regulatory requirements
• Implement therapeutic exercises, activities, ADL retraining, adaptive equipment training, and other treatment interventions
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Educate patients, caregivers, and families regarding treatment plans and functional outcomes
• Complete timely and accurate documentation
• Prepare comprehensive discharge recommendations and summaries
