Tempe Post Acute is seeking a passionate Occupational Therapist to join our in-house therapy team. Responsibilities

• Screen and evaluate patients with cognitive and/or ADL functioning disorders and develop plans of care in accordance with standards.
• Utilize standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Administer OT activities, therapeutic exercises, ADL retraining, modalities, devices, and other OT interventions.
• Assess patient progress and modify treatment plans as appropriate.
• Educate and counsel patients, families, caregivers, and other providers; communicate with supervisors and team.

Key Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care in accordance with regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Administer therapeutic activities, therapeutic exercises, ADL retraining, modalities, assistive devices, and other occupational therapy interventions.
• Assess patient progress and modify treatment plans as appropriate.
• Evaluate treatment outcomes and make referrals when necessary.
• Educate and counsel patients, families, caregivers, and other healthcare providers.
• Communicate effectively with supervisors and interdisciplinary team members regarding patient progress and treatment plans.
• Complete all required clinical documentation and discharge summaries in a timely manner.
