Sweetwater Care, a family-owned long-term care facilitator in Northern Colorado, seeks a compassionate Occupational Therapist to join our in-house therapy team at a midsize facility in Larimer County. You will deliver rehabilitative services to residents with cognitive and ADL needs, collaborating with an experienced clinical staff and administrator.

The role emphasizes evaluation, care planning, documentation, and coordination with families and providers.

Responsibilities

  • Screen and assess patients with cognitive and ADL needs; develop individualized care plans.
  • Provide rehabilitative interventions and therapeutic activities per plan.
  • Document progress and adjust treatment plans in EMR per regulatory standards.
  • Coordinate with therapists, families, and care teams; participate in care conferences.

Job description

Our family owned and operated long term care facilitator is seeking a compassionate and well versedan Occupational Therapist for consideration to add to the inhouse therapy team a midsize facility situated in Northern Colorado, amongst Larimer County. This building is home to a diverse group of long-term residents, cared for by seasoned staff with low staffing ratios, who are stewarded by a well-equipped clinical team and Administrator with years of experience in the geriatric field.

Principal Responsibilities

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

  • Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapist.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure
