Job summary

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Qualifications

• Assesses functional abilities in seniors in skilled nursing or post-acute care.
• Develops evidence-based treatment plans tailored to individuals.
• Delivers therapeutic interventions to promote independence, safety, and quality of life.
• Collaborates with an interdisciplinary care team.
• Maintains accurate and timely documentation of care.
• Educates patients, families, and staff on treatment goals and expectations.

Responsibilities

• Assess functional abilities and safety needs.
• Develop and implement evidence-based treatment plans.
• Deliver OT interventions to improve ADLs and mobility.
• Collaborate with nurses, physicians, and other therapists.
• Document patient progress and adjust plans as needed.
• Educate patients, families, and staff on care plans.
