City Park Healthcare & Rehabilitation in Denver, CO is seeking a PRN Occupational Therapist to join our in-house rehab program. You will screen and treat patients with cognitive and ADL challenges, develop care plans, and deliver skilled interventions.

This role offers a flexible PRN schedule and pay from $42 to $50 per hour based on experience, with benefits for eligible staff.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a plan of care.

• Administer ADL retraining, therapeutic activities, exercises, modalities, and use of devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed based on results.
• Provide consultation to patients, families, and other providers about physical disorders.
• Communicate with supervisor and health team about progress, barriers, and plans.
• Provide discharge summaries per regulatory requirements.

OCCUPATIONAL THERAPIST - PRN - CITY PARK HEALTHCARE & REHABILITATION - DENVER, CO

City Park Healthcare & Rehabilitation in Denver, CO, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery.

We are currently seeking an PRN Occupational Therapist for our in-house rehab program! PRN: Flexible schedule with work-life balance

Pay: $42 - $50/hour based on experience

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.


• Utilizes standardized assessments and evidence-based practice to support clinical interventions.


• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.


• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.


• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.


• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.


• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.


• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.


• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
