Job summary

Sweetwater Care and Rehab is seeking a licensed Occupational Therapist to join our in-house rehabilitation team in California. The role focuses on cognitive and ADL function rehab, patient assessment, treatment planning, and documentation, with emphasis on evidence-based practice and regulatory compliance.

Qualifications

• OT licensure in good standing in state
• Experience in rehab therapy preferred

Responsibilities

• Screens/assesses patients with cognitive and/or ADL disorders; develops care plans per standards
• Uses standardized assessments and evidence-based practice to support interventions
• Provides rehabilitative, skilled, and medically necessary treatment per regulatory requirements
• Delivers ADL retraining, therapeutic activities, exercises, and use of assistive devices
• Monitors progress and adjusts treatments in the clinical record
• Evaluates outcomes and refers as needed based on progress
• Provides counseling to patients and families about physical disorders
• Communicates progress with supervisors and health team members
• Prepares discharge summaries and maintains patient records per policy
• Records treatment minutes accurately and participates in care conferences and trainings
• Supervises and supports occupational therapist assistants
• Adheres to NBCOT/AOTA and state practice acts and code of conduct
• Complies with company policies

Principal Responsibilities

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function, and, when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

• Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops an appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan, including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, assistive devices, etc.
• Assesses patient’s progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
• Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participation in required meetings, including but not limited to patient care conferences, utilization review meetings, rehabilitation meetings, etc.
• Participate in facility and company-required training, in-services, and conferences.
• Able to supervise and support occupational therapist assistants
• Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
• Adherence to Company required policy and procedures and code of conduct.
