Oak View Health & Rehabilitation in Conway, SC is seeking a full-time Occupational Therapist for our in-house rehab program. We offer a $5,000 sign-on bonus and a flexible schedule within a stable, supportive team focused on geriatric recovery.

Licensed OT with ability to screen and treat cognitive and ADL impairments, use evidence-based assessments, and collaborate with the care team to develop and adjust plans of care. New grads welcome!

Qualifications

• Licensed Occupational Therapist (OT) in good standing.
• Experience with cognitive and ADL assessments is preferred.
• Ability to develop and monitor treatment plans and modify as needed.

Responsibilities

• Screen/evaluate patients and develop care plans.
• Utilize standardized assessments and evidence-based interventions.
• Provide rehabilitative treatment for cognitive/ADL disorders.
• Document progress and adjust plans per regulations.
• Discharge planning and family/caregiver counseling.
• Communicate progress with the care team.

Oak View Health & Rehabilitation in Conway, SC is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking afull time Occupational Therapistfor our in-house rehab program!

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

Qualifications

Occupational Therapist license is required.
